When you see this

This approver already decided this request. Each approver decides a request once; a second approval does not count twice and a later rejection does not replace an approval.

Quorum (M-of-N) approval is described in Quorum Approval^*.

Common Causes

• A double click or a client retry sent the same approval twice.
• The approver tried to change an approval into a rejection.

1. Read the request (getTenantQuorumRequest): its decisions list shows what was recorded.
2. Nothing else is needed: the recorded decision stands.
